Do fibroids need to be removed before IVF?
Ultrasound showed a 3 cm intramural fibroid not touching the cavity and a tiny submucosal suspicion. I am 36 planning IVF. What usually needs surgery?
Short answer: Submucosal fibroids that distort the cavity are often removed before transfer. Small intramural fibroids without cavity distortion may be observed. MRI or hysteroscopy clarifies anatomy when ultrasound is uncertain.
Answer #1
Cavity matters most
Fibroids that bulge into the endometrial cavity (FIGO 0–2) are most clearly linked to lower implantation and higher miscarriage risk and are commonly treated hysteroscopically when appropriate. Mapping the fibroid relative to the cavity is more important than size alone.

Answer #2
Intramural fibroids and timing
A 3 cm intramural fibroid without cavity distortion often does not mandate surgery before IVF, though decisions individualise with symptoms and prior failures. If hysteroscopy confirms a submucosal component, removing it before transfer is usually preferred. Allow healing time after myomectomy as advised before stimulation or transfer.

Call your clinic or go to the hospital the same day if you have
- Heavy bleeding with dizziness
- Severe acute pelvic pain
- Fever after myomectomy
