Both tubes are blocked. Is IVF the only option?
HSG showed bilateral block. I am 31. Can surgery open the tubes, or should we go straight to IVF?
Short answer: IVF is the most reliable path for bilateral tubal block. Selected proximal blocks may be considered for tubal surgery in experienced hands, but hydrosalpinx often needs disconnection or removal before transfer because it lowers IVF success.
Answer #1
Why IVF fits tubal factor
IVF retrieves eggs and returns embryos to the uterus, bypassing the tubes. For distal disease with hydrosalpinx, pregnancy rates improve when the damaged tube is removed or clipped before transfer. Discuss imaging details with your specialist.

Answer #2
When surgery is discussed
Young patients with limited proximal obstruction and no significant male factor sometimes consider specialised tubal procedures, accepting ectopic risk and possible need for IVF later. Reversal after prior sterilisation is a different scenario. Avoid unproven 'tube flushing' tourism without clear indications.
