I have endometriosis. Do I need surgery before IVF?
Laparoscopy two years ago showed stage II endometriosis. Pain is controlled. We are moving to IVF. Must I have another surgery first?
Short answer: Not always. Many patients with treated or mild-to-moderate endometriosis proceed to IVF without repeat surgery unless there is a large endometrioma, severe pain or another surgical indication.
Answer #1
IVF can work without repeat surgery
IVF bypasses some pelvic factors by fertilising eggs in the lab. Repeat ovarian surgery can reduce reserve. Ultrasound, AMH, pain level and prior operative notes guide whether surgery adds value or mainly adds risk.

Answer #2
When surgery is reconsidered
Large endometriomas blocking safe retrieval, suspicion of another pathology, or debilitating pain may tip toward surgery. Hydrosalpinx is different and often needs treatment before transfer because fluid can lower implantation rates.

Call your clinic or go to the hospital the same day if you have
- Sudden severe pelvic pain
- Fever after egg retrieval
- Rapidly enlarging ovarian cyst symptoms

Answers are general education for patients in India, not a diagnosis or personal medical advice. Best IVF Centers is a fertility matching service, not a clinic or hospital. Always follow your treating doctor's instructions. In an emergency, go to the nearest hospital.
