Does varicocele surgery improve fertility?
Scrotal Doppler showed a left grade 2 varicocele. Semen analysis is borderline. Should my husband undergo repair before IVF?
Short answer: Varicocele repair can improve semen parameters in selected men but is not automatic before IVF. Decision depends on grade, semen trend, female age and how long you will wait for improvement.

Who may benefit
Men with a clinically palpable varicocele, abnormal semen parameters and otherwise unexplained infertility are most often considered. Subclinical Doppler-only varicoceles have less clear benefit. Improvement, if it occurs, may take several months after microsurgical repair.

Do not delay for female age
If the female partner is older or ovarian reserve is limited, waiting many months may not be wise. Parallel IVF planning while optimising the male factor is common. Surgeon experience with microsurgical technique affects outcomes and complications.
