My husband has a low sperm count. What should we do next?
Semen analysis showed concentration 8 million/ml, motility 25%, morphology 2%. He is 34, non-smoker, desk job. Is ICSI the only option?
Short answer: One semen report is not a final diagnosis. Repeat testing after healthier habits, evaluate reversible causes, then choose among IUI, IVF or ICSI based on the full couple workup.
Answer #1
Repeat and look for reversible factors
Semen parameters vary. A second analysis after 2 to 5 days' abstinence clarifies the baseline. Heat, recent fever, steroids, alcohol, obesity, varicocele and some medicines suppress counts. FSH, LH, testosterone, prolactin and a urology/andrology exam target treatment.

Answer #2
Treatment depends on the couple
Mild to moderate male factor may still allow IUI after optimisation. Severe oligospermia, azoospermia or failed simpler treatments often lead to IVF with ICSI. Female age and tubal status matter equally when choosing the fairest path.

Call your clinic or go to the hospital the same day if you have
- Testicular pain, swelling or trauma
- Blood in semen with fever
- Sudden inability to ejaculate
Want help finding a clinic?
Get matched to one ART-registered clinic after consent — not a directory of calls.
Answers are general education for patients in India, not a diagnosis or personal medical advice. Best IVF Centers is a fertility matching service, not a clinic or hospital. Always follow your treating doctor's instructions. In an emergency, go to the nearest hospital.
