Should I take metformin for PCOS if we are trying to conceive?
My sugar fasting is 108 mg/dl and HOMA-IR is high. Periods are irregular. Will metformin alone help me ovulate?
Short answer: Metformin can improve insulin sensitivity and may restore ovulation in some women with PCOS and metabolic features, but many still need letrozole or other induction. It is not a universal PCOS fertility medicine.
Answer #1
Who is most likely to benefit
Women with PCOS plus insulin resistance, impaired glucose, or higher BMI are typical candidates. Metformin may regularise cycles for some, but pregnancy rates are often better when it is combined with lifestyle change and, if needed, letrozole under monitoring.

Answer #2
Side effects and timing
Start low and take with food to reduce nausea and diarrhoea. Extended-release forms suit many patients. Tell your doctor about kidney issues before starting. Continue preconception folic acid and complete the couple's fertility evaluation in parallel rather than waiting months on metformin alone if age or trying time is already significant.

Call your clinic or go to the hospital the same day if you have
- Severe vomiting or dehydration on metformin
- Lactic acidosis warning signs are rare but urgent
- Low blood sugar symptoms if combined with other diabetes drugs

Answers are general education for patients in India, not a diagnosis or personal medical advice. Best IVF Centers is a fertility matching service, not a clinic or hospital. Always follow your treating doctor's instructions. In an emergency, go to the nearest hospital.
